Stem cells & exosomes

Exosomes vs Stem Cells: What Is the Difference?

Living cells or the signals they release: how they differ and when to use each

Exosomes vs stem cells: stem cells are living cells that can divide, settle in tissue and keep releasing repair signals, while exosomes are the tiny cell-free vesicles those cells release to carry the signals for them. Exosomes suit quick, repeatable treatments for skin and hair with no harvest; stem cells, usually your own mesenchymal stem cells grown in a GMP laboratory, are chosen when a deeper and longer-lasting regenerative effect is the goal.

In other words, exosomes are a product of stem cells, not a type of stem cell. Research over the past two decades suggests that much of the benefit of mesenchymal stem cells comes from what they secrete, and exosomes are a large part of that secretion. For each topic in depth, see our exosome therapy guide, our page on stem cell therapy in Dubai and the overview of stem cell and exosome treatments.

Bioscience Institute works with both every day: GMP-certified laboratories since 2007, a cell factory and clinic in Dubai Healthcare City and a cryobank in San Marino where patients' cells are stored for future treatments.

How do stem cells and exosomes work?

Stem cells work as a living source of repair signals that can stay in the tissue, while exosomes deliver a dose of those signals without the cell. Mesenchymal stem cells (MSCs) are multipotent: they can differentiate into chondrocytes for cartilage, osteoblasts for bone and tenocytes for tendons. Once injected, they move towards damaged tissue, modulate the local immune response and release growth factors, cytokines and exosomes.

Exosomes form inside the cell in small compartments called multivesicular bodies and are released into the space around it. Neighbouring or distant cells take them up by fusion or endocytosis, and their cargo of proteins, lipids, messenger RNA and microRNA changes how those cells behave: less inflammation, more blood vessel growth, more collagen and more cell renewal.

A simple way to picture it

If stem cells are a small repair workshop placed inside the tissue, exosomes are the instructions the workshop sends out. Exosomes carry the message once; stem cells can keep writing new messages as conditions in the tissue change.

Source and preparation

A stem cell treatment starts with a harvest; an exosome treatment starts with a vial. For autologous stem cells, about 20 cc of fat is collected in a short outpatient procedure under local anaesthesia. Our laboratory extracts the mesenchymal stem cells and expands them for about two to four weeks, then checks every batch for sterility, endotoxins, mycoplasma and cell identity (phenotype). Doses range from about 10 to 200 million cells depending on the indication, and the cells not used can be cryopreserved for later treatments.

This is different from same-day SVF (stromal vascular fraction), a mixed cell population in which stem cells are a minority. Expanded MSCs are a purified, counted population, which is why the dose can be planned.

Exosomes are isolated from the culture medium of mesenchymal stem cells grown in the laboratory, then tested for quantity, quality, sterility and endotoxins. Bioscience Institute uses adipose-derived mesenchymal stem cells (ADMSC). Because exosomes contain no cells, the patient needs no harvest and no waiting time, and the preparation does not need to be matched to the patient.

Which conditions is each one used for?

Exosomes are mainly used where a strong signal in the skin, the scalp or a small area is enough, and stem cells where tissue needs deeper and longer support.

Exosome protocols

Stem cell treatments

Exosomes vs stem cells for hair loss

For early thinning, exosomes are usually enough and simpler; for more advanced thinning where follicles are miniaturised but still alive, stem cells from your own fat are designed for a deeper, longer-lasting effect. Exosomes act on the follicle's environment: they support the growth phase, blood supply and a calmer scalp, with sessions that need no downtime.

HAIRSKILL implants your own expanded mesenchymal stem cells into the scalp, where they release growth factors and support tissue regeneration. It works only if the follicle is not irreversibly compromised; first results are visible after about a month and increase gradually, and the response depends on genetics, skin condition, age and lifestyle. It can also prepare the scalp before a hair transplant. If you are weighing a blood-based option too, read exosomes vs PRP.

Exosomes vs stem cells for the face and skin

Exosomes are the usual choice for skin quality, while stem cells come into play when the face also needs volume. Exosomes stimulate collagen, reduce inflammation and improve texture, tone and pores, and they support recovery after microneedling or laser.

Stem cells are most often used in aesthetics together with your own fat: in fat grafting, they support the blood supply of the transferred fat and help it settle. This makes them the option for hollow cheeks, temples or other areas where volume has been lost, while exosomes can refine the skin surface on top.

Exosomes vs stem cells for joints

For joints, stem cells have the longer research history, while exosomes are at an earlier stage. Mesenchymal stem cells can differentiate into cartilage-forming cells and modulate inflammation inside the joint, which is the basis of ORTHOSKILL. The evidence is real but modest: a Cochrane review of stem cell injections for knee osteoarthritis reports a slight improvement compared with placebo, and full structural regrowth of cartilage has not been proven.

Exosomes for joints (EXOJOINTS) aim mainly at inflammation and the tissue environment, and clinical research on them is still developing. For the knee specifically, see stem cell therapy for knees.

Sessions, downtime and when results show

  • Stem cells: two appointments, the fat harvest and then the treatment after expansion. A single treatment is often sufficient, and it can be repeated from stored cells without a new harvest. Expect some bruising or soreness at the harvest site for a few days.
  • Exosomes: same-day treatment with no harvest, given as a short course followed by maintenance. Microinjections typically need no downtime.
  • Results: both act gradually. Skin changes may appear within weeks; hair follows the growth cycle over months; joints are judged on pain and function over several months.

Are exosomes safer than stem cells?

Exosomes carry a lower theoretical risk because they contain no living cells: they cannot divide, multiply or form unwanted tissue, and they have low immunogenicity. Autologous stem cells are also well tolerated because they come from your own body, so rejection is not expected; their risks are mainly those of the harvest and the injection, and they depend on the quality of the laboratory.

With either option, the product decides most of the safety: where it was made, under which standard and with which batch tests. Temporary redness, swelling and tenderness are the usual effects; active infection, active cancer and pregnancy are usual reasons to postpone treatment.

Cost: why stem cells cost more

Stem cell treatments cost more than exosome sessions because they involve a harvest, weeks of GMP laboratory work, cell counting and quality control, a planned dose and often long-term storage. Online medical booking platforms list Dubai MSC packages at USD 3,500 to 27,500. Exosomes are priced per session: public price pages of Dubai clinics (October 2026) range from about AED 999 for a package to AED 2,000 to 5,000 per session.

The fair comparison is the cost of the whole plan. A course of exosome sessions with maintenance can add up, while stored stem cells allow later treatments without a new harvest. For more detail, see stem cell therapy cost in Dubai.

When does combining exosomes and stem cells make sense?

Combining them makes sense when one problem has two layers: deep tissue that needs cells and a surface or environment that benefits from frequent signals. Typical combinations are:

  • a stem cell treatment for the main goal, followed by exosome sessions to maintain the result;
  • fat grafting enriched with stem cells for volume, with exosomes to refine skin texture;
  • home care with exosome cosmetics (XHAIR, XSKIN) between clinical sessions, designed to prolong their results.

Who should choose which?

Choose exosomes for early, surface-level goals and quick sessions; choose stem cells for deeper goals and when you want your cells stored for the future.

Exosomes may suit you if

  • you have early hair thinning or early signs of skin ageing;
  • you want no harvest, no waiting and no downtime;
  • you prefer short, repeatable sessions.

Stem cells may suit you if

  • your goal is deeper: joint degeneration, more advanced thinning with living follicles, or lost volume;
  • you want your cells stored for future treatments;
  • you accept a small harvest and a wait of a few weeks.

FAQ

What is the difference between exosomes and stem cells?

Stem cells are living cells that can divide, settle in tissue and keep releasing repair signals. Exosomes are the cell-free vesicles those cells release to carry the signals, so they act for a shorter time but need no harvest.

Are exosomes stem cells?

No. Exosomes are vesicles released by stem cells, not cells. They contain proteins, lipids and RNA from the parent cell but cannot divide or form tissue.

Are exosomes better than stem cells?

Neither is better in general. Exosomes suit skin, early hair thinning and quick repeatable sessions; stem cells suit deeper goals such as joints, more advanced thinning or volume restoration with fat grafting.

Are exosomes safer than stem cells?

Exosomes carry a lower theoretical risk because they contain no living cells and cannot divide. Autologous stem cells are also well tolerated because they are your own; with both, safety depends mainly on laboratory quality and batch testing.

Which lasts longer, exosomes or stem cells?

Stem cells are designed for a longer effect because they can stay in the tissue and keep releasing signals. Exosomes deliver a single dose of signals, so they are usually given as a course with maintenance.

Do I need surgery for a stem cell treatment?

No surgery is needed. About 20 cc of fat is collected in a short outpatient procedure under local anaesthesia, and the cells are expanded in the laboratory for a few weeks before the treatment.

Can exosomes and stem cells be used together?

Yes. A common plan uses stem cells for the main goal and exosome sessions to maintain the result, or combines stem cell enriched fat grafting for volume with exosomes for skin texture.

Why do stem cell treatments cost more than exosomes?

Stem cell treatments include a harvest, weeks of GMP laboratory expansion, quality control and often storage. Exosomes are a ready laboratory product priced per session, although a full course can add up.
