EXOMALE - Exosome therapy for erectile dysfunction

The EXOMALE injection protocol: assessment, the session, number of sessions, aftercare

Exosome injections for ED, the EXOMALE protocol, deliver concentrated exosomes from mesenchymal stem cells directly into the erectile tissue through fine intracavernosal injections, in a short outpatient session with minimal downtime. The number of sessions and their spacing are set after an assessment of what is causing the erectile dysfunction.

This page is the practical guide to exosome penis injections: the assessment, what happens on the day, whether it hurts, how many sessions are needed, aftercare and how the exosome shot compares with the P-shot. For how exosomes act on the causes of ED, read exosomes for erectile dysfunction; for the wider picture, see our guide to exosome therapy.

Before the first injection: the assessment

Every EXOMALE plan starts with a medical assessment, because the right protocol depends on the cause of ED. The physician typically reviews:

  • your medical history, medications and previous ED treatments, including how you responded to tablets;
  • cardiovascular risk factors such as diabetes, blood pressure, cholesterol and smoking;
  • blood tests, including hormones such as testosterone;
  • a validated questionnaire such as the International Index of Erectile Function (IIEF), the baseline for measuring progress;
  • when useful, a penile Doppler ultrasound to assess blood flow.

If the assessment finds a cause that needs its own treatment, such as uncontrolled diabetes or low testosterone, that treatment comes before or alongside the injections.

What happens during an EXOMALE session

An EXOMALE session is a short outpatient procedure in a private treatment room. It follows these steps:

  1. Preparation: the physician confirms the plan, and the exosomes are prepared for injection.
  2. Numbing: the skin of the penile shaft is cleaned and an anaesthetic cream can be applied.
  3. Injection: with a very fine needle, the exosomes are injected into the corpora cavernosa, the two erectile chambers, at precise points along the shaft.
  4. Pressure: gentle pressure on the injection points limits bruising.
  5. Discharge: after a brief rest you go home the same day.

The exosomes are extracted from human mesenchymal stem cells and tested for quality and quantity. Before use, every product goes through a screening process that certifies sterility, safety for use and endotoxin levels.

Does the exosome penile injection hurt?

The injection usually causes brief pressure or a short sting rather than lasting pain, because the needle is very fine and the area can be numbed first. The injections themselves take only a few minutes.

Small bruises or slight swelling at the injection points can appear and fade within days. Persistent pain, a prolonged erection or signs of infection should be reported to the clinic straight away.

How many exosome sessions are needed for ED?

The number of EXOMALE sessions depends on the cause and severity of ED and on how you respond, so it is set individually rather than fixed in advance. Tissue repair takes time, so sessions are spaced out and progress is reviewed between them.

PhaseWhat happensWhy
AssessmentHistory, blood tests, IIEF score, Doppler if neededFind the cause and set a baseline
Treatment courseOne or more injection sessions, spaced outGive the tissue repeated regenerative signals
ReviewIIEF questionnaire and clinical checkMeasure change against the baseline
MaintenanceOccasional session if neededSupport the result, especially with ongoing risk factors

Men with risk factors that keep damaging the blood vessels, such as diabetes or smoking, should work on them during and after the course, because they decide how long the result lasts.

Recovery and aftercare

Recovery after an EXOMALE session is quick: with minimal downtime, you can return to normal daily activities the same day. Your physician will tell you when to resume sexual activity and intense exercise, usually after a short pause to let the injection points settle.

  • Follow the hygiene instructions you receive at discharge.
  • Expect possible small bruises; they fade on their own.
  • Continue any medication your physician has approved.
  • Keep up the habits that protect blood vessels: exercise, no smoking, good blood sugar and blood pressure control.

Exosome shot vs P-shot (PRP)

The exosome shot and the P-shot are both regenerative penile injections, but they use different material. The P-shot uses platelet-rich plasma (PRP) prepared from your own blood on the day, so its growth factor content varies from person to person and with the preparation. The exosome shot uses exosomes from mesenchymal stem cells, produced and tested in the laboratory, which carry a broader mix of signals, including microRNAs.

Neither is a quick fix: both aim to improve tissue health over time. Our detailed comparison of exosomes vs PRP covers source, preparation, evidence and when each one makes sense.

Combining EXOMALE with other treatments

EXOMALE can be part of a wider plan. Oral PDE5 inhibitors can continue during the course if your physician agrees, low testosterone is corrected when present, and low-intensity shockwave therapy is another regenerative approach some men consider.

For men who prefer a treatment based on their own cells, Bioscience's ANDROSKILL uses autologous adipose-derived stem cells; it is described on our erectile dysfunction treatment page. Men with curvature from Peyronie's disease have their own options on our Peyronie's disease page, and the difference between cell-free and cell-based treatment is explained in exosomes vs stem cells.

Is EXOMALE right for you?

EXOMALE is designed for men with ED of vascular, nerve-related or age-related origin who want a non-surgical, regenerative option, including men who respond poorly to tablets. It is not the first step for purely psychological ED, and it does not replace treatment of diabetes, heart disease or hormonal problems. The physician confirms whether it suits you at the assessment.

What affects the cost of exosome injections for ED

The cost depends on the work-up, the number of injection sessions and the follow-up.

  • Initial assessment and tests, such as blood tests and Doppler ultrasound.
  • Number of EXOMALE sessions in the course.
  • Maintenance sessions, if planned.
  • Combined treatments.

FAQ

How are exosome injections for ED given?

Exosomes are injected with a very fine needle into the corpora cavernosa, the erectile chambers of the penis, after the skin is cleaned and numbed. It is a short outpatient procedure.

Is the exosome penile injection painful?

It usually causes brief pressure or a short sting, because the needle is very fine and the area can be numbed first. Small bruises can appear and fade within days.

How many exosome sessions are needed for ED?

The number is set individually after the assessment, according to the cause and severity of ED. Sessions are spaced out and progress is reviewed between them.

How long does an EXOMALE session take?

The injections take a few minutes; with preparation and a short rest the visit is brief, and you go home the same day.

When can I have sex after an exosome injection?

Your physician will tell you, usually after a short pause to let the injection points settle. Normal daily activities can resume the same day.

What is the difference between an exosome shot and the P-shot?

The P-shot uses platelet-rich plasma from your own blood, while the exosome shot uses laboratory-tested exosomes from mesenchymal stem cells. Both aim to improve tissue health over time.

Are there side effects of exosome injections for ED?

Small bruises, slight swelling or tenderness at the injection points are possible and settle on their own. Persistent pain, a prolonged erection or signs of infection should be reported straight away.

When will I notice a change after EXOMALE?

Changes develop gradually over the weeks and months after the course, as blood vessels and tissue repair. Progress is measured with the IIEF questionnaire against your starting score.
